Resignation with a neutral summative: what to ask for, and what it will not fix
Short answer
Ask for the full text of the summative, the verification of standing, and the competency evaluations to be agreed in advance and attached to the separation agreement, with signed copies given to you and sent unchanged on request. Then negotiate how the separation itself is recorded, because a neutral summative attached to a record of probation and a resignation while dismissal was pending rarely gets a resident placed.
A neutral summative sounds like a win. On its own, it rarely is.
When a program wants a resident to leave, a neutral summative evaluation is often the first thing offered, or the first thing asked for. It matters. But a receiving program, a specialty board, and a licensing board will ask about much more than the summative.

What verification forms actually ask
Training verification and licensure forms commonly ask:
- Dates of training and credit awarded.
- Whether you were ever on probation, remediation, or leave.
- Whether you were investigated or suspended.
- Whether you resigned while an adverse action was pending.
- Whether you left in good standing and were eligible to continue.
A neutral summative answers none of these. The program will answer them from whatever its file says.
Ask for the whole package, in final text
The stronger version of the ask agrees in advance on the full text of:
- The summative evaluation.
- The verification of training and standing.
- The competency evaluations that will be sent with them.
That text is attached to the separation agreement itself. You receive signed copies when you leave. The program commits to sending the same documents, unchanged, whenever it is asked, and names one person to answer verification requests.
Fix the record behind it
A favorable summative attached to a record that still shows probation, and a resignation while dismissal was pending, will not get you placed. Ask at the same time for:
- Probation rescinded, or recorded as successfully completed.
- Any pending recommendation withdrawn, so nothing is pending when you leave.
- Written confirmation of good standing and eligibility to continue training.
- A separation label other than resignation, where your contract allows one. See what resignation changes.
- The program's written position on data bank reporting.
Get credit for your training
Ask for written verification of all training time completed and credit awarded, by rotation, in a form your specialty board will accept. Check your board's continuity rules before you agree to resign.
What a realistic outcome can look like lists every term residents ask for.

Common questions
- What is a neutral summative evaluation in residency?
- The summative evaluation is the program director's final assessment of a resident's performance. A neutral summative describes training completed and competencies without adverse characterization. Residents often ask for one when agreeing to leave a program.
- What should I ask for besides a neutral summative?
- The answers your program will give on verification forms. Ask for the full verification text, probation recorded as completed, nothing pending when you leave, and a separation label other than resignation, all negotiated together with the summative.
- Can my program change my summative after I sign a separation agreement?
- Not if the full text is agreed and attached to the agreement, and the program commits to sending that same text unchanged. A promise to work out the language later is worth very little once you have signed.
